FINANCES:
Payments for professional services rendered is expected and due at each visit. This includes estimated patient portions, co-payments, and deductibles. It is the policy of this office to collect from the parent or person who brings the child for dental services (including a divorce situation). All emergency patients being seen for the first time are required to pay in full at time of visit. For your convenience we accept cash, check, debit cards, all major credit cards, and an in-house finance program offered by Care Credit.

INSURANCE:
We are providers for all PPO plans. If your insurance is not among the PPO plans we are contracted with, you will use your out of network benefits. The benefits from dental insurance vary and are dependent upon what you or your employer pays for your coverage. Because of complex insurance policy variations, we can only ESTIMATE your coverage and CANNOT GUARANTEE it. If you have any questions regarding your benefits please contact your insurance company. As a courtesy to you, we will be happy to file your insurance forms for you. However, our professional services are rendered to you NOT to your insurance company and as such, the financial obligation for dental treatment is between you and this office and is NOT dependent upon insurance.

By law insurance company is required to pay each claim within 30 days. It is YOUR responsibility to see that insurance company makes prompt payment. YOU are responsible for any balance on your account after 30 days regardless of insurance payment. We will be glad to send a refund to you once insurance has paid us.

Please remember:

  • No insurance pays 100% of all procedures.
  • Deductibles and co-payments must be paid by you.
  • Only you have a contract with your insurance company.
  • We are not responsible for how your insurance company handles its claims.
  • We are not responsible for what benefits your insurance company pays on a claim.
  • We do not guarantee what your insurance will pay on a claim.

Please keep us informed of any insurance changes such as insurance company name, address, policy name, policy number, and change of employment.
